Rates and Insurance
The following insurance plans are accepted:
Anthem Blue Cross Blue Shield
United Healthcare UMRTriCare East (approved out of network provider)Aetna
Insurance information must be provided for verification purposes prior to the first session. If insurance is unable to be verified before first session, the private pay rate will apply until insurance is verified.
Private pay rate is $150 for a 50-minute session.
Cancellation Policy
A 24 hour notice is required for cancellations. Any late notice or missed appointment will be charged a $100 fee. Any exceptions will be at the discretion of the therapist.
Payments
Full payment is expected at the time service is rendered. Checks, cash or credit cards are accepted. A credit or debit card will be required to be kept on file following first appointment.
